My go rank is about 13-kyu. Quite often, when I'm playing, my opponent's territory gets too big without my noticing it.
By the time I do notice, it's too late. What should I do to overcome this problem?

I wonder if you have got into the habit of following your opponent around the board, answering every move he or she makes.
Try to focus not just on the local position and the opponent's moves; instead, try to develop the habit of looking at the whole board.
Probably the most difficult thing in go but also the most fascinating is whole-board balance.
It may be difficult at first, but one way to do this is, from time to time, to sit up straight in your chair and consciously survey the whole board; then take a deep breath and make a fresh start in the game.
